PET
ADOPTION AGREEMENT

I HEREBY AGREE TO THE FOLLOWING TERMS AND CONDITIONS PERTAINING TO THE PET ADOPTED BY ME FROM, PET RESCUE ASSOCIATION IN CONSIDERATION OF PET RESCUE ASSOCIATION'S AGREEMENT TO PROVIDE THE UNDERSIGNED WITH THE PET INDENTIFIED HEREIN: 1.
I understand that I am adopting this animal solely as
a household pet, and in no event shall this animal be resold to any person,
broker, breeder, medical institution, research institution, or used solely
as a guard animal, and that in the event that I must ever give up this
pet for any reason whatsoever, I shall notify PET RESCUE ASSOCIATION and
return it to them upon request; in no event shall I give this animal away
to any person, take it to any animal shelter, any veterinarian for euthanasia
or abandon it in any manner or form. 2.
That this pet will be spayed/neutered if it is
younger than breeding age, it shall be altered by the time it is capable
of breeding and a certificate of sterility will be presented to PET RESCUE
ASSOCIATION within 15 days of the date listed herein; if it is of breeding
age, a certificate of sterility will be presented to PET RESCUE ASSOCIATION
within 15 days of the date listed herein.
In no event shall this animal be allowed to breed. 3.
That I shall provide the proper food, shelter and medical
care for my adopted pet. In the
event that I can no longer provide
the necessities for this pet, I shall return the animal to PET RESCUE
ASSOCIATION, and PET RESCUE ASSOCIATION agrees to accept him/her back
into their care. 4.
I fully understand that, except in the case of extreme
emergency, this animal shall be euthanized for medical reasons without
a second opinion from a qualified veterinarian.
PET RESCUE ASSOCIATION will provide the name of such a qualified
veterinarian if necessary. 5.
I agree to notify PET RESCUE ASSOCIATION immediately
if the pet adopted herein becomes lost.
I further agree to notify PET RESCUE ASSOCIATION if said pet dies
of any cause, either natural or unnatural. 6.
That I agree not to allow this animal to roam free beyond
the confines of my enclosed property.
If the animal herein is a cat or kitten, this shall be taken to
mean said animal shall be kept indoors unless leashed. 7.
That I shall provide identification tags for my pet within
48 hours of the date of this agreement. 8.
In the event that I move from the location noted on this
agreement, or if I have any telephone change for any reason, I agree to
contact PET RESCUE ASSOCIATION and notify them of my new address and telephone
number. 9.
I agree that PET RESCUE ASSOCIATION may, at a mutually
convenient time, make a “follow-up” visit in order to ascertain that the
above conditions have been satisfied. 10.
PET RESCUE ASSOCTION
agrees that a 50% refund of your donation will be made on an animal returned
within seven (7) days of adoption when accompanied by this agreement. FAILURE
TO FULFILL ANY OF THE ABOVE TERMS WILL CONSITUTE A BREACH OF THIS AGREEMENT
AND WILL AUTHORIZE PET RESCUE ASSOCIATION TO RECLAIM THIS PET AND/OR TAKE
LEGAL RECOURSE. ALL
ANIMAL PLACED BY PET RESCUE ASSOCIATON ARE, TO OUR KNOWLEDGE, IN GOOD
HEALTH. HOWEVER, WE RECOMMEND
THAT YOU HAVE YOU NEW PET EXAMINED BY YOUR VETERINARIAN.
